Why should we join the campaign?
Get credit for the work you are already doing. Your hospital is already working on a number of the 5 Million Lives interventions, since they are aligned with other national health care initiatives supported by the Centers for Medicare & Medicaid Services (CMS) and The Joint Commission. By joining the Campaign, you can become a part of the Institute for Healthcare Improvement’s national recognition and take advantage of free tools and resources provided to facilitate implementation.

How do we join the campaign?
Participating in the campaign is free and simple. The first step is to submit the 5 Million Lives Campaign enrollment form. Beyond that, hospitals need only submit:

Do hospitals enrolled in the 100,000 Lives Campaign need to re-enroll?
Hospitals do not need to re-enroll but should re-affirm their commitment by updating their hospital profile on the IHI Web site and identifying the interventions they’re working on. By updating your profile and submitting facility mortality data by June 8, hospitals will be considered “fully committed” to the 5 Million Lives Campaign.

Does the 5 Million Lives Campaign include the interventions from the 100,000 Lives Campaign?
Yes. The six campaign interventions from the 100,000 Lives Campaign are included in the 5 Million Lives Campaign. Those interventions are:

The 5 Million Lives Campaign includes six new interventions, including:

Do hospitals have to commit to all of the campaign interventions?
No. Hospitals are only required to adopt one of the twelve interventions; however, there is no limit to the amount of interventions hospitals may work on. Hospitals may already be working on many of the interventions as they are aligned with other national and state health care initiatives.

How should hospitals submit Campaign data?
Hospitals may submit data directly to IHI online at www.ihi.org/extranetng, or hospitals may choose to submit data through one of a few Campaign-authorized "Data Intermediary" organizations, which will then forward the information to IHI. Recorded training sessions and detailed instructions explaining how to use the IHI data submission tool are in the Materials section of the Campaign website. Hospital-specific data will not be published.
